Construction Coordinator, Special Effects Coordinator, Key Grip, Rigging Gaffer | This report is filed daily when construction, pre-rig and/or strike is ongoing | Prod. Office Coordinator | This form offers the construction coordinator or key HODs a mechanism to document and summarize safety conditions, meetings, alerts, equipment or other safety actions. |
Form 4: |
Location Manager | Once per filming location, prior to filming | Prod. Office Coordiantor | When on location, this form serves to identify and document location specific safety concerns. |
Production Report | 2nd AD or SM | Safety meetings are held | Prod. Office Coordiantor | Safety meetings are held to inform cast & crew of anticipated safety concerns. These meetings are documented on the production report. |
Call Sheet | 2nd AD or SM | Safety concerns are anticipated; planned safety meetings | Prod. Office Coordiantor | When safety concerns are anticipated by a production, cast & crew shall be informed ahead of time via the call sheet. |
Form 5: |
Employee's direct supervisor or production medic with assistance from Employee's supervisor | After an incident / injury which requires medical assistance away from the location or studio. | Prod. Office Coordinator and Production Safety Consultant | The completion of this document is required to document the incident. It will be integral to any official investigation and also helpful in identifying the cause and future prevention of similar injuries. |
Form 6: |
This will be issued by the employee's supervisor. This form may be requested by production management or the production safety consultant. | When an employee has failed to follow any of the following safety compliance measures. Employee safety guidelines codes of safe practices, safety bulletins, call sheet, or safety meeting advisories, or failed to perform their job as common sense or job knowledge would dictate. | Employee notices of violation are issued by the employee's supervisor. This form should be filed by the production office & forwarded to the prod. safety consultant. | This notice becomes a record for possible future disciplinary action. Copies may be forwarded to labor relations if deemed necessary. |
Form 7: |
1st AD or SM with assistance from personnel knowledgeable in the area of concern | When an unsafe condition or a safety hazard exists and the situation cannot be corrected immediately | Prod. Office Coordinator and the Production Safety Consultant | When an unsafe condition is identified, two actions should always occur. 1) Notify all concerned employees (document on production report) and 2) determine a corrective plan of action (document with notice of unsafe condition & action plan). |

Forklift, Telehandler and Aerial Lift Operators | Every day for every forklift, telehandler, and aerial lift. | Prod. Office Coordinator | This form should be used to document the daily lift inspections by the operators of all forklifts, telehandlers and aerial lifts. |
