Authorized Entrant Duties
The entry supervisor shall ensure that all authorized entrants:
- Are knowledgeable on the potential hazards that may arise during the entry and work operations including information of the symptoms and consequences of exposure.
- Have been trained in the safe and proper use of all equipment required on the job.
- Communicate with the attendant as necessary to allow the attendant to monitor the status of the entrants and to allow the attendant to alert all entrants of the need to evacuate the area.
- Alert the attendant whenever any symptom of exposure to a hazardous situation or if a prohibited condition is detected.
- Exit the confined space immediately if any of the following situations arise:
- An evacuation order is given by either the attendant or entry supervisor.
- The entrant recognizes any symptoms of exposure to a hazardous situation.
- The entrant detects a prohibited condition.
- An evacuation alarm is activated.
- An alarm sounds on the air monitoring equipment.
Attendant Duties
The entry supervisor shall ensure that all attendants:
- Are knowledgeable on the potential hazards that may arise during the entry and work operations including information of the symptoms and consequences of exposure.
- Are knowledgeable of the potential behavioral effects of hazard exposure by the entrants.
- Keeps an accurate count of the number of authorized entrants in the space.
- Remains outside the confined space during all entry and work operations unless relieved by another authorized attendant.
- Communicates with the entrants as necessary to allow the attendant to monitor the status of the entrants and to allow the attendant to alert all entrants of the need to evacuate the area.
- Monitors the work area for potential hazardous situations.
- Orders an evacuation of the confined space immediately if any of the following situations arise:
- The attendant recognizes any symptom of exposure to a hazardous situation.
- The attendant detects a prohibited condition.
- The attendant detects a situation outside the space that could endanger the authorized entrants.
- The attendant cannot properly perform all required duties.
- An evacuation alarm is activated
- An alarm sounds on the air monitoring equipment.
- The attendant shall summons rescue and emergency services if necessary.
- Keep unauthorized employees and visitors away from the space.
- Be knowledgeable in the proper use of all required equipment.
- Be knowledgeable in the procedure for performing nonentry rescues.
- Must not perform any duties that may interfere with the primary duty of the attendant which is to monitor and protect the authorized entrants.
Entry Supervisor Duties
The Safety Program Director shall ensure that each entry supervisor:
- Is knowledgeable of the potential hazards that may arise during the entry and work operations including information of the symptoms and consequences of exposure.
- Verifies that all required entries have been made on the permit, that all required air testing has been conducted and that all procedures and equipment specified on the permit are in place before signing the permit and allowing the entry to begin.
- Assures that the entrants and attendants know the appropriate entry procedures.
- Terminates the entry and cancels the permit when necessary.
- Verifies that rescue services are available and that the means of summoning them are operable.
- Removes unauthorized individuals from the area during entry operations.
- Determines that all entry operations remain consistent with the terms of the entry permit at all times during the entry.
